Creative Horizons Application Form

This form is for anyone eager to share their creative work at the Creative Horizons Showcase. By participating, you can get noticed and open doors for your future. Plus, you'll receive complimentary entry to the event and savour a delicious meal.

Please complete form below and send payment of $40 by e-transfer to africentricarts.info@gmail.com or pay online by clicking here. Registration is $35 for members and $40 for non-members. Following submission, you will be redirected to make a payment to finalize the registration process. Thank you for joining us on this creative journey.

Refund Policy

Kindly be aware that we will not process any refunds within 5 days leading up to the commencement of the event.

Photo /Video Consent

I, hereby grant permission to Africentric Arts its employees, partners, representatives, and agents to take photographs and videos of me/my child during the Kids Fashion Show event on [Event Date]. I understand that these photographs and/or videos may be used for promotional, marketing, and advertising purposes, including but not limited to, social media, websites, brochures, posters, and other promotional materials. I hereby release and discharge Africentric Fashion, the Yatta Rose Children’s Foundation, and their respective representatives from any and all claims, actions, and demands arising out of or in connection with the use of said photographs/videos, including, without limitation, any and all claims for libel or violation of any right of publicity or privacy.
